Summary                                                                                

The Diabetic Educator provides training and education for Diabetic patients enabling them to become self-managing of their disease. Plans, organizes and delivers education or MLHS patients and community members as a licensed, professional RN or Registered Dietician.  The Diabetes Educator utilizes nursing skills (assessment, planning, intervention, evaluation) in a systematic approach to the direct and indirect care of diabetic patients and their families/caregivers and in alignment with MLHS and the Diabetes Self-Management program's mission.  Supports and participates in the assessment, implementation, monitoring and evaluation of quality activities in accordance with the MLHS quality program.

Will consider RN or CDE for a 1.0 FTE - (80 hrs per pay period)- Day Shift Position 

Job Duties

  • Develops, implements and provides education program for Diabetic patients.
  • Keeps and records documentation related to Diabetic training as required by guidelines from the American Diabetic Association.
  • Develops, Provides, & implements up to date information Diabetic clients and takes responsibility for planning of program.
  • Keeps self-educated and informed on current and changing trends and techniques and integrates them into her lesson plans for all educational offerings.
  • Supports the MLHS Mission/ Vision/ & Values and incorporates this example into her education plans and through her daily actions and performance of duties.
  • Functions as a positive professional role model by mentoring less experienced staff, coaching and sharing skills.
  • Identifies safety risk concerns and actively attempts to resolve or report the concern to appropriate person.
  • Follows Infection Control guidelines and intervenes when observing someone failing to follow safety or infection control guidelines.
  • Participates in the control of cost effective services by proper use of equipment, supplies and takes responsibility for reporting equipment that is not functioning properly.
  • Exercises good judgment and seeks out answers to questions.
  • Works independently with minimal supervision.
  • Promptly addresses customers' requests & concerns and attempts to exceed their expectations whenever possible.
  • Is courteous and respectful when speaking to customers both external & internal.
  • Follows facility policies related to confidentiality or sensitive information.
  • Actively participates and plans community events promoting wellness, diabetes education and diabetic resources for the community.
  • Is available to provide information necessary for clinic and hospital staff in the area of diabetes and the diabetic patient.
  • Works closely with MLHS and outside providers providing referral for DSME services.
  • Provides consults as ordered by the ordering provider.
  • Provides in-services for clinic and hospital staff as requested.
  • Diabetes disease and treatment process.
  • Incorporating nutrition management and physical activity into lifestyle.
  • Use of diabetes medication safely.
  • Blood glucose monitoring, interpreting and using results as a pattern management.
  • Prevention, detection, ad treatment of acute and chronic complications.
  • Developing strategies to address psychosocial issues.
  • Developing strategies to promote health/change behavior.
  • Attends and participate in appropriate diabetic education to ensure that MLHS is offering the most up to date information and education to patients and staff members.
  • Establishes and maintains links to diabetes resources by identification of patient needs.
  • Participates in the development of curriculum and training materials.
  • Provides referral to other members of the DSME care team in order to make available optimal diabetes care for the patients.
  • Establish/maintains a diabetic registry and incorporates into clinical management of diabetic patients.
  • Contributes to, and participates in a continuous quality improvement process to measure DSME program and to identify and address opportunities for improvement.

Required Education and Experience

  • Graduate of an accredited school of nursing with current licensure as a Registered Nurse in the State of Minnesota or Registered as a Licensed Dietician.
  • Completes at least 15 CEUs per year pertaining to diabetes per American Diabetes Association recognition requirements.
  • Knowledge of Nursing practice, policies, procedures, and medical ethics equivalent to years of experience.
  • Knowledge and at least 1 year of experience with diabetic patients.
  • Keeps current with new diabetic changes.
