 The position provides high quality patient care.  Demonstrates good patient and family assessment skills.  Plans and implements treatments of individuals and families.  Facilitates treatment psycho-diagnosis, treatment planning, group treatment, individual treatment, family treatment, and discharge planning.  Functions well as a member of the multidisciplinary team and participates in community relations for the Sr. Care Unit.

 Ensures that there is community knowledge of the Geriatrics Mental Inpatient Unit at Mille Lacs Health System.  Visits all current referrals and new referrals.

1.0 Days- Full Time with Full Benefits-Travel within the area will be expected as a Community Liason Coordinator

Job Duties

  • Engages in timely, efficient discharge planning.

  • Works in close conjunction with patient, family, nursing staff and attending physician.

  • Shows knowledge of referral/placement process and available community resources.

  • Conducts 10 day – 2 week follow-up via phone.

  • Conducts individual psychotherapy sessions as applicable.

  • Communicates with family members for the purpose of continuity of care.

  • Conducts group therapy sessions.  Displays theoretical knowledge of subject matter and effective communication skills.

  • Completes social evaluations on assigned patients as per policy.

  • Interviews, diagnostic and report writing skills.

  • Completes comprehensive progress notes in a timely manner.

  • Ensures notes are completed within 24 hours.

  • Communicates with physicians in person, phone, Telemedicine techniques.

  • Functions cooperatively as a member of the treatment team.

  • Communicates effectively with families and agencies, conveying a positive professional image.

  • Brings to Program Director's attention significant issues related to program functioning.

  • Utilizes feedback regarding performance and use supervision to improve performance.

  • Responds to behavioral emergencies utilizing CODE GREEN techniques.

  • Assists with Milieu Management.

  • Acts as back up for ADLS.

  • Follows up on all inquiry calls to the unit regarding services.
    • Prioritizes assignments and meets deadlines; Flexing priorities with department need.

    • Protects client confidentiality and models ethical behavior.

    Marketing of Services:

    • Demonstrates self-motivation, self-direction requiring minimal supervision.

    •  Provides a clear understanding of our services and potential customers.

    • Makes contacts with referral sources such as nursing homes, assisted living, physician offices and other organizations serving the elderly.

    • Presents in-services as suggested/requested to professional and community organizations on topics related to aging mental illness.

    • Completes intakes on potential patients, may coordinate admission process.

    • Provides unit tours, participates in health fairs and other community events as approved.

    • Designs written documents describing program services, including but not limited to: unit website, newsletter, admission information, workshop notices, unit brochure, newspaper articles.

    • Maintains accurate and timely census reports, marketing reports as approved by manager.

    • Maintains up to date referral source files, seeks new referral sources.

     Preferred Education and Experience

    • Master's degree as a Social Worker preferred, with 5 years mental health experience, one field placement experience or at least one year experience working with Adult Psychiatric patients, and master's level social worker supervision.

     Required Education and Experience

    • Bachelor's degree in Social Service field required.

    • Current Licensure
    • Clinical skills, ability to work productively and effectively, ability to function well as a member of a multidisciplinary team, and ability to represent the program well in interaction both inside and outside the host hospital.

    • Must have knowledge of EMR.

    • Computer skills – Outlook, Excel, PowerPoint, etc.
