Develops, organizes, implements, and evaluates the social services program in accordance with the principles of social work practice and state and federal laws regulating social services in long term care. Coordinates admissions to long term care and discharges from facility, as well as effectively facilitating optimal psychosocial functioning of residents.  Works effectively with members of the interdisciplinary team to enhance resident care and to safeguard their rights and privacy.

Required Education and Experience

  • Baccalaureate Degree in Social Work. Current licensure in the State of Minnesota as a social worker.
  • Optional: Certification in Basic Life Support.
  • Two or more years of experience as a social worker in healthcare setting, long-term care and Home Care/Hospice preferred
  • Must successfully complete criminal background check as required by the State of Minnesota.
  • Must demonstrate knowledge and skills necessary to provide care appropriate to the residents served in long term care.
  • Must demonstrate knowledge of the principles of growth and development over the life span.
  • Must be able to assess and interpret data relating to the resident's status in order to identify the resident's age specific needs.
