Assists with processes to provide care for patients.  Tasks to include: and other duties as assigned.  Communicates with patients and their families, Providers, Nurses, pharmacies, ancillary department staff and outside agencies.  Utilizes Clinical leadership in problem solving.

Job Duties

  • Provide nursing care utilizing the components of the nursing process.
  • Demonstrates effective interviewing and integrates new or current techniques to assist in obtaining physical/psychosocial information from the resident/family as directed by a Registered Nurse through the care planning process and use of required federal and state instruments.
  • Identifies and actively supports the role of resident advocate/liaison by communication and documentation of resident information to the proper resources/agencies as directed.
  • Functions as a positive role model in professional relationships and interactions by utilizing and sharing leadership skills appropriate to the role of a Licensed Practical Nurse within the nursing home.
  • Adheres to restorative care philosophy by incorporating principles into direct care provided to residents on a daily basis.
  • Identifies needs and concerns involving safety/risk management and Quality Improvement issues. Participates in the research, action, and evaluation of proposed resolution/interventions.

Required and Preferred Education and Experience

  • Minnesota Licensure as LPN (required)
  • Minnesota Licensure as RN (preferred)
  • Basic Life Support/CPR certification (required)

Additional Eligibility Qualifications

  • Demonstrate the knowledge and skills necessary to provide care to the resident population served in LTC facility.
  • Demonstrate knowledge of the principles of growth, development and aging, from adolescence through the end of life, including death and dying.
  • Assess and interpret data relating to the resident's status in order to identify the resident's age specific needs.
  • Good interpersonal and communication skills (verbal, nonverbal and listening)
  • Computer skills including Word, Excel and Outlook.
